What to bring to a DOT physical: a driver's checklist

A DOT physical can go from stressful to routine when you arrive prepared. This checklist covers the documents, details and expectations that keep the exam moving, plus what the certificate actually requires.

The checklist

Arrive with these and the exam runs smoothly:

  • Valid government-issued driver's license
  • A current list of every medication you take, with doses
  • Glasses, contact lenses or hearing aids if you use them
  • Records for any condition you are being treated for, such as blood pressure logs or sleep apnea compliance reports
  • Your most recent Medical Examiner's Certificate, if you are renewing
  • The names and contact info of your treating providers, if relevant

What gets measured

The FMCSA exam covers vision and hearing, blood pressure and pulse, a urinalysis screen (for sugar and protein, not drugs), a review of your health history, and a physical examination of the systems the federal form specifies. Everything is recorded on the Medical Examination Report, form MCSA-5875.

If you pass, you receive the Medical Examiner's Certificate (MCSA-5876), valid for up to 24 months. The examiner may issue a shorter certificate, for example 3, 6 or 12 months, when a condition such as blood pressure needs monitoring. The certificate expires at 11:59 PM on the date listed, and you must carry it, or a copy, while driving.

Honesty is the fastest route to certification

Drivers sometimes worry that disclosing a condition will cost them their card. In practice, the opposite is usually true: an examiner who has the full picture can certify you with supervision, while a surprise on the record means delays and extra visits. Bring real information, ask what the standards are, and you leave knowing exactly where you stand.

If you are turned down, the examiner is required to explain why and what would allow you to qualify, such as treating a condition or providing additional records. Many drivers return to certification once the issue is managed.
